Arranging the financial, custodial, and medical care for a disabled dependent is a difficult planning challenge. There are certain issues that may affect your dependent's eligibility for continued Health Fund benefits. This information has been provided to assist you in maximizing any benefits that may be available to your disabled dependent under Health Fund.

Benefit Plan Checklist
- Older dependent children who are physically or mentally disabled may be considered dependents if they become disabled before turning age 26. Once your child reaches the age of 26, you must provide the Health Fund with a completed Disability Application Form
. These documents may be required annually thereafter by the Health Fund.
